Hearing: Per mandate under the guidelines of the Arizona Department of Health Hearing Conservation Program, hearing screenings are given to students in grades preschool, K, 1, 3, 5, 7. Special education students are screened on a yearly basis. Vision: Per Arizona guidelines, vision screenings are conducted in grades preschool, K, 1, 3, 5 and 7. All special education students are screened on a yearly basis. Scoliosis: Students are screened for scoliosis in grades 6 and 7. Other Screenings: Optional dental screening may be conducted in some elementary grades. Optional blood pressure screening may be conducted in grades 5 and 9.

School is no place for a sick child. Please do not send your child to school if he/she has diarrhea, vomiting, a fever, rash, deep cough, or a communicable disease such as chickenpox, strep throat, pink eye or lice. You will be notified if your child becomes ill enough to be sent home from school, sustains more than a minor injury, or has an emergency. It is the parent’s responsibility to make arrangements to pick up an ill or injured child as promptly as possible. Please have someone listed on your emergency form in case you are not available or cannot leave work. Any student having any of the following will need to be sent home: vomiting, having two or more bouts of diarrhea, evidence of possible pink eye, a rash, or with a temperature of 100.0 degrees. Children are not permitted to walk home when ill or injured per district policy. If a child is sent home with a fever, diarrhea or vomiting, they are not to come to school the following day. They must be symptom free and/or fever free for 24 hours before returning to school. They must also have been on the use of medication before Dr. prescribed antibiotics (such as for pink eye or strep throat) for 24 hours before returning to school. Other conditions may require a Dr. note stating the student is not contagious and may return to school.

It is imperative that you notify the Health Office if your child has a medical condition such as diabetes, seizure disorder, asthma, heart disease, allergies, or any other type of physical/emotional/mental disability. This will assist the Health Office staff and the school’s staff to better meet the needs of our child. This information should also be included on the emergency/health history forms. Please return these forms as soon as possible once school has started. Don’t forget to make the Health Office aware of any changes in the home, employment or emergency contact numbers you may have.

Crutches, wheelchairs and assistive walking devices used by students for an injury require a doctor’s note specifying the injury and length of suspected need. Crutches, wheelchairs and assistive walking devices will not be dispensed from the Health Office at the student or parent request. These items must be supplied by the parent/guardian with a doctor’s note. This information helps the Health Office Staff to make emergency plans in the event of fire, the need to evacuate or potential problems with ambulating. Students should be assessed for appropriate technique in use of device and supplied with teacher notes/passes and/or elevator key as needed.

Arizona State Law ARS 15803 states that all children must have up to date immunization records on file at school to be able to attend. Verification for all immunizations must include the child’s name, birth date, type of vaccine, date of vaccine, and name of doctor/agency administering vaccine. Records must be signed or stamped by the doctor/agency providing the immunizations. A previous school record is also acceptable (does not include preschools). Records must be provided before a child can attend school. Many immunization clinics are available in the area that are free. Please remember to take all the child’s immunization records with you to the clinic for their staff to review. Information may be obtained from this site. Clinic Your Health Office staff will be able to tell you which immunizations your child needs in order to attend school.
